Refer for Formal Assessment
If by the third School Action Plus review the childs progress is deemed not to be satisfactory, the Headteacher on the advice of the SENCO and the Educational Psychologist should consider advising the LEA that a statutory assessment may be necessary to meet the childs educational needs.
In exceptional circumstances, this may happen in less time.
There needs to be:
written information on:
  • educational and other assessments, eg specialists, educational psychologists;
  • views of the parents and the child;
  • childs health;
  • social services / educational welfare involvement;
written information of:
  • school action under the three stages; (Sheet A,B);
  • IEPs for the child (all should be retained);
  • Regular reviews and their outcomes; (review proforma for each review);
  • Involvement of other professionals (recorded on Sheet B).
Routes for referral:
  • referral by the school / another agency;
  • a formal request for an assessment by a parent.

The steps in the whole process are:
  • considering whether a statutory assessment is necessary
  • making an assessment
  • drafting the proposed statement or not to make a statement
  • finalising the statement
Future action
  • continue at School Action Plus Stage, set new targets if no statement is issued;
  • move to Statement Stage.
Child has a legally binding statement of Special Educational Needs.
