If by
the third School Action Plus review the childs progress is deemed
not to be satisfactory, the Headteacher on the advice of the
SENCO and the Educational Psychologist should consider advising
the LEA that a statutory assessment may be necessary to meet
the childs educational needs.
In exceptional circumstances, this may happen in less time. |

eg specialists, educational psychologists;
- views of the parents and the child;
- childs health;
- social services / educational
welfare involvement;

stages; (Sheet A,B);
- IEPs for the child (all should

- Regular reviews and their outcomes;
(review proforma for each review);
- Involvement of other professionals
(recorded on Sheet B).
