A The Initial Concern List (Teacher responsibility) |

| This
form is found near the front of the class special needs file.
This list is compiled after the first two weeks of the school
year and is also used as concerns become apparent during the
year. As the name suggests, it is a register of pupils for whom
the teacher has an initial concern because they believe the pupil
is falling behind the majority of pupils in the class. Pupils
who appear on this list will automatically be put on the special
needs register as At Risk, unless evidence comes into school
to show that they are on a higher stage of the register. These
pupils access the teaching of the rest of the class through differentiation.
This stage will be for a maximum of 2 terms. Those pupils for whom concerns exist, will be recorded on Form B The Pupil With Special Needs Class Record. |

| It is important to be as precise as possible regarding the area of concern as it will help to identify future assistance. The childs strengths can also be helpful when considering strategies and resources to employ. | |||

| Once identified and recorded on The Pupil with Special Needs Class Record the concern should be reviewed every term. The pupils has now been identified as having special needs which are being monitored, initially on the AT Risk register. At this stage, parents must be informed by the class teacher, and the nature of the special educational provision should be recorded. | |||
| The pupil remains
the responsibility of the school and the class teacher involved. Recorded on Sheet C Class Summary Sheet |

| At Risk Stage | |||
| The At Risk Stage review should be carried out at the end of the term, before the SENCO collects in the SEN files. The future action following the review should be recorded on Sheet C, Class Summary Sheet. | |||
| The review should
focus on: progress made by the child; effectiveness of the special help; future action. |
